I'm a new patient. Can I fill out paperwork ahead of time?
Absolutely! Click the link at the bottom of this page to download and fill out our new patient forms before your appointment.
What do you charge for consultations?
Our consultations are $30
What is your cancellation policy?
We require a 24-hour cancellation notice for all treatments.
What can I expect on my first visit to Bella Luz?
We want every single person to come through our doors feeling welcome and comfortable. You will be asked to fill out New Patient paperwork before your appointment begins. During your consultation, we will take the time to answer any questions you may have and work with you to create a personalized treatment plan.
Is there anything I should know before receiving injectables?
Avoid blood thinning over-the-counter medications such as aspirin, ibuprofen, Motrin, and Aleve. Avoid supplements, including St. John's Wort, primrose oil, and vitamin E. Avoid consuming alcohol 24 hours prior.
If you have previously suffered from facial cold sores, there is a risk that the needle punctures could contribute to another eruption of cold sores. Please let us know if you are prone.
Are your services covered by any form of health insurance?
We do not accept any form of insurance.
What forms of payment do you accept?
We take cash, check, card as well as offer financing through Care Credit.
